Immigration and medical licensing are separate
Canada introduced a 2026 Express Entry category for eligible physicians with at least one year of full-time Canadian medical-doctor experience in the previous three years, reserved up to 5,000 federal PNP spaces for doctors, and 14-day work-permit processing for qualifying nominated physicians.
Immigration approval does not authorize a person to practise medicine. Foreign credentials must be assessed and the physician must obtain the licence required by the medical regulator of the province or territory where they will work.
IRCC reports that 31% of family physicians were internationally trained in 2024, one in four health-care workers are immigrants, and more than 11,000 health-care workers arrived through economic immigration in 2024.

Express Entry — Canadian physician category
For a physician eligible for one of the three Express Entry programs who has at least one year of full-time Canadian physician experience in the last three years. Invitations may also come through general or program-specific rounds.

Key issues to verify
- Credential recognition and the exact licensing pathway of the province or territory where the doctor intends to practise
- Eligibility for an underlying Express Entry program before relying on the new Canadian-experience physician category
- At least one year of full-time Canadian physician experience in the last three years for that dedicated Express Entry category
- The correct physician NOC, lead statement and duties; the 14-day work-permit measure is limited to NOC 31100, 31101 and 31102
- A genuine job, job offer, provincial support letter, nomination, designated employer or community recommendation where the chosen route requires it
- Language, education or ECA, recent work experience, settlement funds or exemption, admissibility and intention to live in the selected province or community
- Spouse or common-law partner, dependent children and eligible dependent grandchildren may be included in the permanent-residence application

A practical sequence
- 01
Start credential recognition
Have foreign medical credentials assessed and contact the medical regulator for the intended province or territory. Licensing can take time and should begin early.

Separate Canadian experience from no Canadian experience
Use the dedicated IRCC pathway pages to compare the physician category, other Express Entry rounds, PNP, AIP, RCIP and FCIP.

Confirm the underlying program
Verify every minimum requirement, NOC, work period, language result, education document, funds rule and location requirement before creating a profile or accepting an offer.

Obtain route-specific support
Secure the eligible job offer, provincial nomination or support letter, designated-employer offer, endorsement or community recommendation required by the selected route.

Apply for permanent residence with family
Submit the complete federal application and include eligible accompanying family members with consistent identity, relationship and admissibility documents.

Use faster work-permit processing only if eligible
A nominated doctor must meet the specific NOC, offer, support-letter and completeness rules, including an upfront medical exam and timely biometrics where required.

Important points
- Medical licensing and immigration selection are separate decisions; permanent residence alone does not permit medical practice.
- The special Express Entry physician category still requires eligibility for one of the three Express Entry-managed programs.
- The 5,000 PNP spaces are an allocation to provinces and territories, not 5,000 automatic approvals or a direct federal application stream.
- Fourteen-day processing is a service standard for an eligible complete work-permit application, not a guarantee of approval or issuance in exactly 14 days.
- Québec does not have a PNP and uses its own selection and medical-licensing systems.
- Health-care workers who are not medical doctors should review the broader health-care Express Entry category and other federal, provincial and regional options.
